Land Disposal Restriction (LDR) Forms

LDR forms are notifications by the generator that the shipped waste can or cannot be disposed in a landfill without proper treatment. They may also be referred to as "Land Ban Forms".

An LDR form must accompany the hazardous waste manifest for each hazardous shipment to a Calgon Carbon reactivation facility. Generators are not required to use the standard Calgon Carbon LDR form. Any LDR form which meets the Federal Standards is acceptable.

Calgon Carbon's LDR form requires the following basic information:

Generator Name

Carbon Acceptance Number

Manifest Document Number (five character tracking system devised by generator)

The generator must then indicate the Land Ban status of the spent activated carbon. Most generators will select status code "A. Restricted Waste Requires Treatment." Hazardous waste codes should be indicated in the first space after "A."

For waste codes F001 through F005, as well as California List constituents, page 2 of the LDR form shows the applicable treatment standards. This page must be included, and waste constituents should be circled as appropriate.

For waste codes F039, D001, D002 (not accepted at Calgon Carbon), and D012 through D043, the subsequent pages of the LDR form show the applicable treatment standards. These pages must be included. Follow the directions on the form for identifying underlying hazardous constituents.

For all other waste codes, the generator completes the table on page 1 of the LDR form:

Column 1: Applicable Waste Codes

Column 2: NWW (for non-wastewater)

Column 3: Treatment Standard from 40CFR Subpart D – 268. Generator can either list a numeric standard for the hazardous waste constituent, or state "Thermal Treatment – Spent Carbon."

Column 4: Subcategory Description – Usually not applicable. Generator must sign and date the LDR Form.
